Since you enlisted at MEPS, have you changed bank accounts?

Are you taking ANY prescription medications?

Since you enlisted at MEPS, have you had ANY injury, mental health issues, or medical procedure since enlistment?

Since you enlisted at MEPS, have you received ANY new tattoos, body piercings, brandings, or scarification?

Since you enlisted at MEPS, have you been ticketed, arrested, charged, cited, held or detained by ANY law enforcement?

Do you have ANY personal hardships that could interfere with your Ship Date to Training?

Do you understand that if you fail to report to Training on your reserved Ship Date that you will be reported as Absent Without Leave (AWOL) and processed through the Justice Court System? A warrant will be issued for your arrest and charged with AWOL (MS Code 1972 07-7-59).
